I hot glued a tassel and beaded top to thread a loop through, you could also fold some thread in as you glue it together, or use a needle to sew through the top. 
 
You will recieve a zip folder with 3 pdfs, one mint, one pink, and one black & white version.
They are sized at US Letter 8.5 x 11"
Print quality will depend on your printer, paper, and settings. Mine is set to high quality on matte photo paper.
